I put in a message box that I have to click OK on before the trigger runs.&lt;br/&gt;

&lt;p&gt;On pressing a custom toolbar button the trigger is launching an application and auto-types it's login data. Everything's fine so far.&lt;/p&gt;
&lt;p&gt;Sometimes later I want to search something in Keepass. I click in the search field, type something and hit Return. But instead of searching, Keepass behaves, as if the Toolbar Button was pressed again, i.e. executing the configured trigger actions for this button. &lt;/p&gt;
&lt;p&gt;You can watch this in action in the attached video.&lt;/p&gt;
&lt;p&gt;If you look closely, you'll see, that after pressing the "VDI" button, the button keeps being "selected" (blue) and even keeps being selected, when returning to the keepass window, clicking into the search box and typing. Obviously, in this state the Return key press is processed by the button element instead of the search box.&lt;/p&gt;
&lt;p&gt;I can't reproduce this reliably every time I'm using the toolbar buttons, but it happens regularly.&lt;/p&gt;
&lt;p&gt;Generally, this issue can be reproduced with all toolbar buttons, it doesn't have to be the first one necessarily.&lt;/p&gt;

Pressing Ctrl+Shift+U to copy the URL to the clipboard causes NVDA to freeze momentarily. The duration of the freeze scales with the number of entries in the currently focused group.&lt;/p&gt;
&lt;p&gt;Steps to Reproduce:&lt;/p&gt;
&lt;ol&gt;
&lt;li&gt;Open a database.&lt;/li&gt;
&lt;li&gt;Navigate to a group that contains a large number of entries (e.g., 1000).&lt;/li&gt;
&lt;li&gt;Focus on any entry within that group.&lt;/li&gt;
&lt;li&gt;Press Ctrl+Shift+U to copy the URL to the clipboard.&lt;/li&gt;
&lt;/ol&gt;
&lt;p&gt;Expected Behavior:&lt;br/&gt;
The URL is copied to the clipboard promptly with no noticeable delay or screen reader interruption, regardless of how many entries are in the group.&lt;/p&gt;
&lt;p&gt;Actual Behavior:&lt;br/&gt;
NVDA freezes for approximately one second before responding. The freeze duration appears to be proportional to the number of entries in the group. For example:&lt;br/&gt;
• ~10 entries in the group: URL copies almost immediately, no noticeable freeze.&lt;br/&gt;
• ~1000 entries in the group: NVDA freezes for roughly one second.&lt;/p&gt;
&lt;p&gt;Environment:&lt;br/&gt;
• KeePass version: 2.61&lt;br/&gt;
• NVDA version: 2025.3.2&lt;br/&gt;

I've never used that feature before, so I didn't know what was going on. &lt;p&gt;There is no change on 2.61 that would make it suddenly enter your credentials on a right click. It is possible your mouse is being weird.&lt;br/&gt;
Try highlighting an entry and pressing the "context menu" key, usually next to the right Ctrl key. Does it bring up the context menu? Can you select "Perform Auto-Type" with the keyboard?&lt;/p&gt;

How did you go from 2.58 to 2.61?&lt;/p&gt;
