Crash when inserting SURFACE from data

Inserting a surface into QMATPLOT canvas aborts the
execution of the software.

The following message appears after crash in the
command line when trying to insert a surface:

qmatplot: ksmatrix.h:137: double
KSMatrixImpl<ELEMENT_TYPE>::value(int, int) [with
ELEMENT_TYPE = double]: Assertion `col < m_cols' failed.
Aborted

Also, segmentation fault appears when trying to create
a surface from an already existing AXES.

The problem happens in versions 0.4.1 and 0.4.2.

Unable to create/open data sheets

Runing YellowDogLinux 3.0 for PPC.

gcc version 3.2.2-2a installed.

QmatPlot compiles with no problems.

Yet when trying to create a new work sheet or import
anyone, the selection menu remains gray. Qmatplot does
not all the activation of these menus.
